World Champion Tae Kwon Do offers a Three Week Trial with no obligation. This trial offers two classes per week and includes a free uniform that is yours to keep. Tae Kwon Do is a physical activity that includes running, jumping, kicking, punching and may not be suited to some existing physical ailments. By signing this form you are stating that you are in good physical condition and that you understand the risks involved in any sport. You agree to follow the rules of our school, and agree not to hold this school, the landlord, its owner (Master J.B. PARK), or any of his employees liable for any injury incurred here.

I also understand that I can sign up for regular classes with World Champion Tae Kwon Do any time within the first ten (10) days of this trial program to be eligible for having the one-time Membership Fee waived. Signups after the first ten (10) days will be charged the one-time Membership Fee.
